12 Month subscription botox

Service Description

By purchasing this subscription you will be charged £65 per month for 12 payments. You are entitled to up to 4 botox appointments (and reviews). Each botox appointment will be for up to 2 areas, plus one mini-tox area (value £230 x 4 = £920) Additional areas, or mini-areas can be added at an additional £40 per area or £30 per mini-area on the treatment date(s). Each botox appointment must be at least 12 weeks after the last (this doesn't include review appointments that fall 2 weeks after initial botox appointments). All appointments must be held within 12 months of the initial subscription purchase. You will receive a subscription renewal reminder 7 days prior to any payments being processed. When purchasing this package you will: 1. Purchase this package from my online store (here!) 2. Be given a randomly generated code, shown on the webpage and emailed to you (IMPORTANT - SAVE THIS EMAIL) 3. Use that code to redeem immediately using quick link provided, or redeem it over time. 4. When you redeem your code it will deduct appointments booked from the balance of your package. Main areas: frown, forehead, crows feet Mini-areas: lip flip, smokers lines, sad smile, pebble chin, bunny lines, brow lift, gummy smile If you are unable to complete all appointments in the allocated time frame, or are medically unable to have the treatments - you will still be charged until the end of the 12 month period. Please contact Jo in this instance, where possible a gift-voucher will be issued in replacement of the treatments. Rescheduling appointments - please avoid this as much as possible, normal terms and conditions will still apply. Therefore if you cancel with less than 48 hours notice you will still have the allocation deducted from your package. You can reschedule each appointment once, with more than 48 hours notice without a fee being due. Should you 'no-show' an appointment, your allocation will still be deducted from the balance of your package and payments will still be taken in full.
